Patient Portal Web Guide

We have established an internet-based patient portal to help effectively and securely manage your healthcare information. Whether you are a first-time patient, or a returning one, there are features for everyone, such as refilling prescriptions, viewing medical records, contacting doctors, and filling out necessary forms before appointments. The content in this document will guide you through the secure portal’s most frequently-required functions. Accessing the Patient Portal

An account must be created by our staff before you can use or access the patient portal.

Activate Your Patient Portal

  1. Log in to your email on your computer
  2. Look for an email from “Patient Portal”.
    • Note: the subject line should read, “Welcome to Your Chester County Eye Care Associates LLC Patient Portal”
  3. Open the email and take note of your username and the practice URL
  4. Click Activate Account
    • Note: the button will not work after 72 hours of receiving your email, so try to activate your account as soon as possible.
  5. Enter your date of birth and last name, and create a password
  6. After creating your password, you will be prompted to a log in page.
    1. If it asks you for a practice URL, please use: cceyecare.ema.md
  7. Enter your username and password, and click Login

Log in to Your Patient Portal

  1. Go to cceceye.ema.md
    • Note: Make sure that you are under patient login, not provider
  2. Enter your username and password, and click Login 

Reset Your Password

  1. On the log in page, click Forgot Password
  2. Enter your username, date of birth, and last name
  3. Click Request Email
  4. Go to your email and look for an email from “Account Team”.
    • Note: the subject line should read, “Password Reset from Your Electronic Health Record”
  5. Click on the link provided in the email
    • Note: The link will not work after 24 hours of receiving your email
  6. Enter your date of birth and last name to verify your identity
  7. Enter your new password and submit. You will then be prompted to your patient portal

 

Preparing for your Visit

Update Your Medical History

  1. Once logged into the patient portal, select My Health in the navigation bar at the top of the screen.
  2. Look through the different sections and enter your information. Some of these sections include:
    • Contact Info: View basic contact and demographic information
    • Insurance and Pharmacy: View your insurance information and add or edit pharmacies
    • Medications: Add or edit your medications list
    • Allergies: Add or edit your allergies
    • Past Medical History: Add or update your past medical history, ocular history, social history, and family medical history
    • Implantable Devices: View your current implantable, eye-related medical devices

 

Add Your Preferred Pharmacy

  1. Select “My Health” in the navigation bar at the top of the screen.
  2. Click “Insurance and Pharmacy”
  3. There are two options to enter your pharmacy
    1. Add Surescripts Pharmacy (try this first): Search for registered pharmacies that are able to send and receive electronic prescriptions
      1. Select Add Surescripts Pharmacy
      2. Use the Filter tool to find your pharmacy by entering a combination of its phone number, name and zip code.
      3. Locate your pharmacy in the list of results by verifying the name and address
      4. Click Store Name to keep your pharmacy on-file
    2.  Add Manuel Pharmacy: Manually enter your pharmacy’s information
      1. Select Add Manuel Pharmacy
      2. Enter as much information about your pharmacy as possible, including the name, address, phone number, and zip code
      3. Locate your pharmacy in the list of results by verifying the name and address
      4. Click Save

 

Managing Your Appointments

The patient portal allows you to access your medical records, past and upcoming appointments, and medical summaries from your visits.

Review Previous Appointments

  1. Select Appointments in the navigation bar at the top of the screen.
  2. Select Past Appointments
  3. Look under the Visit Date section of the table and select the date that you would like to view
    • Note: If the visit link is in plain black font, that means the summary is not ready to be viewed
  4. Your browser’s PDF viewer should open the document for you to download or print

View Upcoming Appointments

  1. Select Appointments in the navigation bar at the top of the screen.
  2. Select Upcoming Appointments, where you will be able to see the date and location of your appointment, as well as which doctor you will be seeing.

Request an Appointment

  1. Select Appointments in the navigation bar at the top of the screen.
  2. Select the blue Request Appointment button
  3. Answer the four required questions, which ask for your visit reason, preferred doctor, and preferred date and time
  4. Click send to make sure our office gets your message

Viewing Your Records

The Patient Portal gives you access to viewing your medical records and eyewear prescriptions.

View Eyewear Prescriptions

  1. Select Appointments in the navigation bar at the top of the screen.
  2. Select Eyewear Prescriptions
  3. Click anywhere on the corresponding line to download a PDF copy of your eyewear prescription

View Test Results

  1. Select Appointments in the navigation bar at the top of the screen.
  2. Select Results
  3. Click anywhere on the corresponding line to download a PDF copy of your test results

View Billing Records

  1. Select Billing in the navigation bar at the top of the screen.
  2. Click anywhere on the corresponding line to download a PDF copy of your billing statement

 

Making Balance Payments

Patients are able to make secure payments towards their balance through the Patient Portal

Pay Your Bill

  1. Select Billing in the navigation bar at the top of the screen. You will be able to view your account balance and billing statements.
  2. To pay, click Make a Payment.
  3. Input the amount you would like to pay, and press Continue
  4. Select Billing in the navigation bar at the top of the screen.
  5. Input your credit/debit card information. If you would like to save your card on-file, select the checkbox that says Remember for next time
    • Note: If you already have a card on file, you can either select a saved card or pay with a new card
  6. Select Pay
  7. Wait until the payment process is complete before closing or refreshing the screen. Once the page updates, you can with select View my Receipt or select Close

Note: You can also pay your bill by going to Chester County Eye Care’s website and selecting Pay My Bill.

 

Communicate with Our Office

The Patient Portal provides a secure, HIPPA-compliant method of communicating messages with our office.

If you are experiencing a medical emergency or need immediate assistance, call 911.

Send a Message to Our Office

  1. Select Messages in the navigation bar at the top of the screen.
  2. Select Compose Message
  3. Enter a recipient in the To As you start to enter their name, it will narrow the list down to make your search easier
  4. Enter the subject of your message in the Subject field
  5. Enter your message in the Message field
  6. When you are ready to send your message, click Send

 

Read and Reply to Our Messages

  1. Select Messages in the navigation bar at the top of the screen.
  2. To read a message, click anywhere on the corresponding line
  3. After reading a message, you can select Reply or Reply All
  4. Enter your response in the Message field, then click Send
